Photo release form to be signed and included with payment.

Photos submitted electronically should be at least 300 dpi. Minimum acceptable is 150 dpi. Once form is submitted we will provide an email address to send the photo.

If you cannot send a photo electronically, we will make arrangements to have the photo scanned and returned to you.
